Forms Library

If you need a form, this is the place to find it. You can download extra release from liability forms for other teachers and classrooms that plan on participating, as well as other forms.

The following two forms are what you need to participate in the Water Monitoring Day event:

• Release of Liability Form (San Diego, CA only)
  Print and complete this form for each participant in your group. Then, fax it to 858-695-9560.
  
• World Water Monitoring Day 2008 Data Sheet
  Log your results on this form.
  
• If you plan to submit more than 12 sites, you may instead want to download the batch form template, an Excel spreadsheet instead of having individual data sheets for each site.
• To be reimbursed for bus transportation to your water monitoring site, download WWMD-TransporationReimbursementInvoice.doc (San Diego, CA only), and save it to your desktop so it's easy to find. Launch MS Word and open the invoice you downloaded. Fill out the necessary areas, print it out on your school letterhead and sign it. The invoice MUST be printed on your school letterhead or it will not be accepted and you will not be reimbursed.

With the exception of the batch form template, all forms in our Forms Library are in Adobe Acrobat Portable Document Format (PDF). You will need the freely available Adobe Acrobat Reader utility to open the form. If you don't already have the Adobe Acrobat Reader utility, download it from the Adobe Systems web site. Then, double-click the executable icon and follow the on-screen prompts to install it to your hard drive.

The Forms Library will regularly be updated as we need to edit existing forms, delete any that become obsolete or add new forms. So, visit often.
